Our Enablement services provide the ideal care solution and environment to help regain crucial skills and abilities to perform day-to-day activities. Our fully qualified and friendly staff are here to provide the on-site training and development of the different skill sets, through a combination of treatment programmes and supportive care.
Our Enhance service offers 24 hour residential care for patients from our local hospitals who are not yet ready to go home, but no longer need to remain in an acute setting. The service is designed to help get people back into their own home or a care home rather than have them waiting in hospital until they are well enough to return to full independence. The earlier releases from hospital allow residents to regain some of the comfort and independence that they may have lost, while still receiving around the clock care. This then eases the transition out of hospital, while retaining full confidence in the well-being of your loved one.
This short term care option is made up of a range of services that will help you to get home after a hospital stay, or to prevent a hospital admission. An average stay lasts from 14 to 21 days. All patients will be assessed by the therapy team within a few days of admission, and then goals will be discussed and an individual therapy plan put in place. You will have the support of our care team during your stay, as well as a range of visiting professionals who will be on hand to assist you in your recovery,

End of Life
We understand that at such a stressful and sensitive time, good nursing care and trustworthy support can make a huge difference to how you cope with a terminal illness. We support residents and their families with planning ahead for their end of life. This is a process called advanced care planning. It enables residents to think about and express wishes on how they would like to be cared for during the final stages of their life. Our dedicated staff receive regular end of life training and work in close collaboration with the community teams of GP’s, palliative nurses, and district nurses to ensure that your care needs and wishes are met.
